How do we design, implement, monitor and evaluate programs?
Our renewed focus on impact influences how we design, implement, monitor, and evaluate programs. To guide us in this process, we created a Theory of Change for our organization, showing the change that we envision happening in the communities that we support, as well as the steps towards achieving this change. These steps include the activities that are part of how we work, the short- and long-term outcomes or expected results of our programs, and the impact or broader social change that we hope to contribute to as an organization.

Improved behavior so that people use the available resources and their knowledge and skills to take care of their own health.

Increased empowerment, so that communities are able to take ownership of and sustain the work to address health disparities.
Activities
1) Connecting communities to primary healthcare and other resources (such as health screenings, referrals, specialty follow-up services, and preventative services).
2) Educating and training community health workers and community members with a focus on health literacy.
3) Supporting the development of tools and solutions that help to prevent diseases.
4) Engaging and educating our network of global health advocates to support with human and financial resources where and when needed.
